Cupid's bow lip reshaping is very popular in Thailand. However, many people do not realize that surgery is only one option. Injectable treatments are another increasingly popular approach. Here are the differences between the methods.
1. Lip Reshaping with Injectable Treatments (Lip Augmentation)
This approach may suit people who want a more defined central lip tubercle and fuller lips at the same time. It is commonly considered for lips that are thin or lack volume, or for patients who prefer to avoid lip surgery. Commonly used options include dermal fillers and the patient's own fat.

Dermal Filler Injections
Advantages
- A convenient procedure with quickly visible results
- Absorbable when an appropriate, approved biodegradable filler from a reputable manufacturer is used
- Generally minimal downtime
Limitations
- Quality products can be relatively expensive
- Uses a manufactured material rather than the patient's own tissue
Autologous Fat Grafting
Advantages
- Uses the patient's own tissue, avoiding allergic reactions to the fat itself
- Can also be used to restore volume in other facial areas
- Fat that successfully establishes a blood supply can remain long term
- May be more economical, depending on the amount and areas treated
Limitations
- Results and fat survival vary according to individual factors and the doctor's technique
- Swelling and bruising may last approximately 7–10 days

2. Lip Reshaping Surgery
Surgery may suit patients seeking a lasting change in lip shape, or those with congenital or injury-related lip deformities. The surgical approach depends on the amount of existing lip tissue, with patients generally divided into two groups.
2.1. Thick Lips: Direct Surgical Reshaping
For thicker lips, the surgeon removes a carefully planned portion of lip tissue to create the desired Cupid's bow contour, then closes the incision with sutures.

2.2. Thin Lips
For thin lips, the surgeon aims to remove as little tissue as possible. A transposition lip flap may be used to move fat or muscle tissue from the sides toward the center of the lip, creating a more defined central tubercle. However, if the lips are too thin, an injectable augmentation treatment may be more appropriate.
